Payment and Insurance Payment is expected at time of service. We accept cash, check, or major credit cards. Other payment plans, including insurance, must be arranged prior to your appointment. Allied Psychophysiology offers discounts and affordable payment plans for neurofeedback and biofeedback treatment and self-pay clients. If you wish to use health insurance, please fill out the client insurance form (downloadable from the Client Forms page). (Call the office for a password to download the file.) Allied Psychophysiology is a Preferred Provider for the following health insurance plans: Blue Cross Blue Shield (although not the HMO plans), Cigna, Cigna Behavioral Health, Medicare, Medicaid, and TRICARE. Effective July 1 2008, we no longer participate with Magellan and MedCost If you wish to file with another plan, our office will provide you with the standard medical claim form (CMS-1500) for your visit.
